Annual reviews


An Education, Health and Care Plan, also called an EHCP or EHC Plan, must have a formal annual review within 12 months of the final plan or the last review. The annual review is an opportunity to review progress, raise concerns or suggest changes if the content of the plan are no longer appropriate and need to be amended.

The young person is encouraged and supported to contribute to the review and take ownership over their Plan. RISE offer a variety of Inclusive ways to capture their thoughts, feelings and aspirations using Padlet, visual drawings, reports, verbal contribution or recorded conversations.

All stakeholders will be encouraged to meet to review the targets, provision and placement so that the review truly reflects the Education, Health and Care needs of the young person.

We encourage an early annual review to help the young person to identify, agree and implement significant changes to their plan without waiting for the next annual review.
